Required Credentials and Certifications

• Current Respiratory Therapist License with primary source verification within 30 days prior to assignment start
• Ascension Respiratory Therapy Contract Worker Onboarding Education Attestation (6 hours pre-hire training with signed attestation)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score)
• Ascension Biometric Consent
• Ascension Non-Nursing Contingent Worker Pre-Hire Modules/Attestation (Illinois)
• Ascension Skills Checklist (clinical skills checklist, completed within one year of start, updated annually)

Health and Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; annual update required
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within one year, and completed TB questionnaire; updated annually
• 2 Step PPD OR IGRA: First PPD within 1 year, second within 30 days of start; or negative IGRA within 90 days prior to start
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually; includes testing for 12 specified substances with MRO review if needed

Professional and Background Verification

• License/Certificate Verification: Must be current and verified; submission required
• Driver's License/State ID: Must be valid, match address on file, and comply with compact license rules if applicable
• Ascension Profile (Resume or Professional Profile): Submission required
• Ascension 2 Professional References: At least two professional references required prior to first assignment
• Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and other checks; completed within 30 days of first assignment and updated annually
• Ascension EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days
• Ascension Education Verification: Verification of relevant education within 90 days of start; rerun if break in service > 90 days
• Ascension Confirmation of Assignment: Provided upon initial assignment and updates for extensions; must reflect accurate start/end dates
• Ascension Employment Verification: Third-party verification for past seven years; updated annually or if break in service > 90 days

• Respiratory Therapy Contract Worker Form is completed onsite by the facility; no need to upload prior to start
• Some onboarding documents and attestations must be uploaded by the agency once completed
• TB Questionnaire and other health documents must be updated annually
• Flu vaccine exemptions require mask wearing at all times during flu season
