Job Opportunity: Physical Therapist (PT) - Travel Position

Location: Prestonsburg, Kentucky
Contract Length: 12 weeks

Job Description

• Must have 2+ years of inpatient Physical Therapy experience in an acute care setting.
• Coverage includes inpatient acute care and skilled nursing facility (SNF).
• Kentucky license required (KY or compact for KY ARH facilities).
• Basic Life Support (BLS) certification required.
• Orientation occurs bi-weekly.
• Candidate must reside within 100 miles of the facility to be considered.
• Assignment start date is the date needed to start.

Required Certifications & Licenses

• BLS (Basic Life Support) – Required for onboarding.
• Kentucky PT License or Compact License – Required for onboarding.
• MMR (Measles, Mumps, Rubella) Vaccination or Positive Titer – Required prior to start.
• Hepatitis B Vaccination Series or Declination – Required prior to start.
• Influenza Vaccine (seasonal, October-March) – Required prior to start or approved exemption.
• Health Statement/Annual Physical – Must be signed by MD, DO, PA, or NP within 1 year prior to start, stating clear to work without restrictions.
• Varicella Vaccination or Positive Titer – Required prior to start.
• Tdap Vaccination – Valid for 10 years; declinations accepted.
• TB Quantiferon Blood Test – Within 1 year prior to start.
• ARH 20/20 Drug Screen – Must be completed within 30 days of start.

Professional Requirements

• Minimum 2+ years of specialty experience required (acute care PT).
• 2 professional references required:

  • 1 must be a charge or higher.
  • 1 must be a supervisor reference within the last year.
• Background checks must be current (within 30 days prior to start) and include:

  • 7-year county criminal history, nationwide criminal, SSN trace, national sex offender, SAM, OIG checks.
• Submission packet must include:

  • Cover sheet form, resume, skills checklist (clinical only), driver’s license, license/certification verification in one PDF.
• Annual updates required for many credentials and background checks.
• Skills checklist, competency exam, and core competencies (HIPAA, safety, infection control, etc.) must be current within 1 year prior to start.
• Kentucky Vulnerable Adult Maltreatment Registry verification required within 30 days prior to start.
• ARH Employee Health Attestation Form must be completed prior to compliance deadline.
• Assignment confirmation form must be uploaded within 24 hours of offer acceptance.

• Prior travel experience is not explicitly required for this position.
• Candidate must not be submitted to more than one job at a time.
