Job Opportunity: Licensed Practical Nurse (LPN) - Evenings

Job Description

• Provide routine daily nursing care and other services according to each resident’s assessment, preferences, and care plan.
• Collaborate closely with the RN Manager to manage day-to-day operations.
• Facility size: 160 beds.
• Resident-to-nurse ratios to be discussed during the interview.
• Must have current BLS and ACLS certifications.

Required Qualifications

• Valid LPN license for Iowa.
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
• Completion of a skills checklist within the last 12 months.
• Current health statement/physician statement within the last 12 months.
• Required immunizations and health screenings including:

  • COVID-19 vaccine (declination accepted)
  • MMR (2 vaccines or positive titer; booster or declination accepted)
  • Hepatitis B (2 vaccines or positive titer; booster or declination accepted)
  • TB test (negative test within past year or appropriate documentation if history of positive)
  • Varicella (2 vaccines or positive titer; booster or declination accepted)
  • Tdap (valid for 10 years; declinations accepted)
  • Influenza vaccine during season (vaccine or facility declination accepted)
• Fit test for approved respirators completed prior to start.
• Drug screen (9-panel) within 30 days of start.
• Background check including state and federal screenings within 30 days of start.
• Verification of professional license and education.
• Professional references (2 required, including 1 supervisor reference).
• Driver’s license or state ID current and valid.
• Completion of required questionnaires and attestations prior to start.

• Prior travel experience is not explicitly required.

• This is a travel assignment with a 12-week contract length.
