Setting: Outpatient/Inpatient split

Job Details

• Position involves working both outpatient and inpatient physical therapy.
• Shared housing is available (no guests or pets allowed).
• Must have own transportation.
• Flu vaccine is mandatory.

Required Credentials & Compliance

Licenses & Certifications

• Physical Therapist License: Primary source verification required within 30 days of start date. MA Conditional Licenses are not accepted.
• BLS Certification: Required for onboarding (AHA preferred; Red Cross accepted).
• Resume: Must include past 7 years of work experience with city and state.
• References: Two recent references required, at least one supervisor or above.
• Skills Checklist: Required for submission.

Health & Immunizations

• Flu Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
• MMR: Documentation of vaccination or proof of immunity required.
• TB Test: Documentation of TB skin test or negative blood test within 3 months of start date.
• Health Statement: Physician statement within 365 days of start date.
• Hepatitis B: Strongly preferred but optional.
• Tdap: Preferred, vaccine within last 10 years.
• Respirator Medical Clearance: Required if fit testing is necessary for the role.

Background & Screening

• Criminal Background Check: Within 45 days prior to start date, including international searches if applicable.
• Massachusetts CORI Check: Within 30 days prior to start date.
• SSN Trace: Must be run with criminal check covering last 7 years.
• National Sex Offender Check: Within 30 days of start and updated annually.
• Exclusion Checks: OIG, GSA, MassHealth & NH Medicaid exclusion lists checked within 30 days of start and annually.
• Education Verification: Verified through background check company.

Additional Requirements

• MGB Confirmation of Assignment/POI Form: Required upon acceptance and for any start date changes.
• Candidate Submission Coversheet: Required for submission.

Additional Notes

• Candidate must have flu vaccine.
• Shared housing requires signing a housing agreement.
• No pets or guests allowed in shared housing.
• Must have own transportation to work site.
