Overview

Location: Charlevoix, Michigan
Setting: Critical access hospital
Unit: 12-bed MedSx unit
Team: Works with CNAs, RNs, and a critical care-trained house supervisor
Contract length: 13 weeks
Shift: Nights, variable nights, 19:00 - 07:30, 12-hour shifts
Hours: 36 hours/week; no guaranteed hours; overtime after 40 hours
Weekends: 2 weekend shifts every 2-week period
Holidays: Not required
On-call: Yes, low census on-call is used when census dips
Dress code: Scrubs, no specific color

Patient Assignment

Average assignment of 4-5 patients, with a maximum of 6
Expected to care for high-acuity patients or float to high-acuity units

Required Qualifications

Valid RN license in Michigan
Med Surg / Telemetry experience: 1 year of experience, within the last year
BLS issued by American Heart Association or American Red Cross, active at the candidate's start date
ACLS issued by American Heart Association or American Red Cross, active at the candidate's start date
Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Experience working with cardiac patients
Experience working with general surgery patients, including post-op care
Experience working with orthopedics patients
Experience working with urology patients
Driver's license
Experience with cardiac or critical care drips, including titratable or cardiac IV medications
Experience with respiratory support (such as heated high-flow)

Onboarding Requirements

Licensing and background: Primary source license verification; fingerprinting; driver's license or state ID; 7-year national criminal background check (including all aliases, criminal history search, national sex offender database search, and FACIS Level 3, with no pending counties accepted), completed within 90 days of start; OIG, OFAC, and national sex offender searches, completed within 90 days of start; Social Security number verification (trace and Death Master File not accepted); I-9 completed
Education and employment verification: Education verified by third party or diploma; employment verified by third party or agency-verified list of previous positions, covering at least 2 employers spanning at least 2 years, matching the resume
References: 2 professional references from a manager, supervisor, or charge role
Health and vaccinations: MMR and varicella proof of vaccination or positive titer (varicella history not accepted); influenza vaccine required November 1 through April 30, with no declinations (medical or religious exemptions allowed if approved and provided by the agency on agency letterhead); TB test (negative T-spot or QuantiFERON within 6 months, or 2 negative skin tests within 6 months; positive or abnormal results require a 2-view chest X-ray and TB screening questionnaire); health statement completed within 1 year of start; drug screen results within 90 days of start
Competency and testing: Core competency exam (OSHA), completed at hire and repeated only after a break between contracts; skill-specific competencies; skills checklist with self-assessment; Ishihara-compliant color vision exam
Documents and forms: Facility badge photo (shoulder/head, plain background); facility profile (resume, skills checklist, contact information) submitted with application; emergency contact form; health system agreement signed and sent within 24 hours of accepting the offer; Social Security card uploaded within 24 hours of accepting the offer
