Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Long Term Care (Evenings)

Job Overview
- Provide continuous observation of residents, noting changes in mental and physical well-being and communicating with supervisors and physicians as needed.
- Administer medications and treatments as ordered, documenting accurately using electronic systems.
- Supervise Licensed Nursing Assistants (LNAs) according to New Hampshire Board of Nursing regulations.
- Assist and assign nursing staff resident care consistent with their abilities and scope of practice.
- Accompany physicians on resident rounds and facilitate communication during 60-day reviews.
- Administer oxygen therapy and gastrostomy/jejunostomy feedings per facility policies.
- Perform wound care, including knowledge of wound vacs.
- Monitor unit activities such as therapy appointments, ambulation lists, vital signs, and nutritional documentation.
- Oversee infection control practices and maintain a clean, hazard-free environment.
- Admit, discharge, and transfer residents with proper documentation.
- Complete 24-hour reports, review new orders, and maintain communication with nursing leadership regarding resident and staff issues.
- Communicate with residents’ families to address concerns and obtain necessary consents.
- Manage assigned unit(s) during shifts under direction of Nursing Supervisor or Director of Nursing.
- Follow state and federal regulations for controlled medication storage, usage, and documentation.
- Assist with orientation, education, and evaluation of nursing staff; recommend disciplinary actions as needed.
Required Qualifications
- Active Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in New Hampshire.
- Minimum of 3 years of current experience within the last 6 months.
- IV certification required (must be willing to obtain during onboarding if not already certified).
- Willingness to float to other units where experienced.
- COVID-19 vaccination required; no declinations or exemptions accepted. Proof of vaccination must be provided.
- Must reside more than 50 miles from the facility; local candidates within 50 miles will not be accepted.
- Previous travel experience is highly preferred.
- Experience working in rural or small facility settings is highly preferred.
Additional Information
- Candidates must have their own transportation due to the rural location and limited public transit and housing options.
- Dress code: Matching scrubs (top and bottoms).
- Overtime and callback pay policies apply (details available upon request).
- Orientation hours are not billable.
- Cancellation policy allows up to 3 shift cancellations per 13-week contract.
- Annual seasonal flu vaccine required or declination with mask-wearing during flu season.
- Timekeeping is electronic; travelers clock in/out at the facility, and timecards are uploaded digitally.

- Provides 24-hour nursing care in a Medicaid-licensed Intermediate Care Facility and Medicare-approved Skilled Nursing Facility.
