  • Criminal Background Check within 60 days covering all locations lived/worked/schooled in past 7 years
  • EPLS/GSA/SAM check within 30 days of start

• Health Requirements:

  • Physical Exam within 1 year of start
  • Negative TB test (QGOLD or 2 Step PPD within 60 days, updated annually)
  • TB Questionnaire if past positive, updated annually
  • Immunizations:

    • 2 MMR Vaccines or Positive Titer (Rubeola, Mumps, Rubella)
    • Hepatitis B Series, Titer, or Signed Declination
    • Varicella: 2 Vaccines, Positive Titer, or History of Chicken Pox (required by agency)
  • Drug Testing: Negative drug screen within 60 days of start (includes marijuana, cocaine, PCP, amphetamines, opiates, etc.)
  • Flu Vaccine or Medical/Religious Exemption during flu season (Oct 1 - Mar 31)
  • COVID Vaccination preferred (initial series)
