- Negative TB or Chest X-Ray (within 12 months; annual updates required)
- MMR Vaccines or Positive Titers for Rubeola, Mumps, Rubella
- Hepatitis B vaccination or signed declination

Employment History and References
- Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
- Minimum of 2 references covering 1 year of employment within last 3 years in comparable specialty/setting (6 months for CNAs)
- References must be from supervisors; charge RNs acceptable if direct supervisors

- Badge Photo (passport style with neutral background) required
- Completion of Providence WorkRamp Modules (Core, Specialty, Regional, and Annual Regulatory) as assigned based on profession and region
