Job Opportunity: PTA SNF (Physical Therapist Assistant - Skilled Nursing Facility)

Location: Grass Valley, California
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months)
Shift Schedule:

• 5 days per week, 8-hour day shifts
• May include occasional weekend coverage and floating

Job Description

• Start Date: ASAP (flexible for later starts)
• 36 hours guaranteed per week
• May consider new graduates
• 1 year of experience in Skilled Nursing Facility (SNF) specialty preferred

Licensure & Certification Requirements

• California License in hand
• ACLS (Advanced Cardiac Life Support) certification from American Heart Association or American Red Cross
• BLS (Basic Life Support) certification from American Heart Association or American Red Cross (current, clear copy required)
• PALS (Pediatric Advanced Life Support) preferred
• State License verification required

Required Documentation & Compliance

• Government issued photo ID (Driver’s License or State ID)
• Emergency Contact Form (required prior to start)
• Background checks including:

  • 7-year residential/work/school history
  • Sex Offender Registry check
  • EPLS/GSA/SAM (annual)
  • HHS/OIG (annual)
• Competency exams as applicable (Medication Test/Pharmacology and general Competency Exam)
• Physical exam within 1 year of start
• Negative TB test or Chest X-Ray within 1 year (with specific guidelines for past positives)
• Immunizations or proof of immunity for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella (Chicken Pox)
• 9 Panel Drug Screen within 1 year of start
• Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)

References & Skills

• 2 professional references required for travel, allied, or per diem acute care roles
• 1 professional reference required for per diem non-acute care roles
• Annual skills checklist required

Experience Requirements

• Minimum 1 year of experience in SNF specialty preferred (new grads considered)
• No explicit prior travel experience required
