Job Description

• Provide home health physical therapy assistance with a focus on:

  • Assessment
  • Cardiopulmonary/Circulatory care
  • Documentation
  • Use of equipment and devices (crutches, gait belt, hospital bed, rolling walker, shoulder slings, standard walker, supplemental oxygen delivery)
  • Orthopedics, Neurology, and Oncology patient populations
  • Home Exercise Program implementation
  • Lifting and transfer techniques
  • Medication knowledge
  • Patient and family education
  • Recognizing signs & symptoms of physical, sexual, and psychological abuse

• Patient population includes cardiopulmonary, geriatrics, med/surg, neuro, ortho, and oncology specialties.

• Home skilled visits: 6-8 visits per day (minimum 6 points per day, may increase at manager’s discretion).

• Work schedule includes every other weekend.

• Visits and hours are self-scheduled based on office needs.

• Must have a valid driver’s license and personal vehicle for home visits.

Required Qualifications

• Valid Physical Therapy Assistant license in Florida
• Minimum 2 years of Physical Therapy Assistant experience, including experience within the last year
• Experience working in a Home Health setting
• Experience working with Geriatric patients
• BLS (Basic Life Support) certification
• Proficient in assessment, cardiopulmonary/circulatory care, documentation, use of equipment and devices, home exercise program implementation, lifting and transfer techniques, medication knowledge, and patient/family education

Compliance and Documentation Requirements

• Valid driver’s license active throughout assignment.
• Background checks including:

  • Standard 7-year history check
  • EPLS/GSA/SAM
  • HHS/OIG
• Education verification and foreign equivalency evaluation if applicable.
• Florida AHCA Level II fingerprint clearance required prior to start.
• Physical exam within 6 months of start date.
• Negative TB test or chest X-ray within 10 weeks of start (or within 1 year if prior positive PPD).
• Annual TB questionnaire required.
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, series, or declination)
  • Varicella
• Standard 9-panel drug test within 30 days of start.
• Respirator medical clearance if respirator use is required.
• References: 2 references from the past 2 years required.
• Employment application and skills checklist must be submitted.
• Color blind testing (Relias Color Blind Exam) required prior to start.
• Medication and competency exams as applicable.

Additional Information

• Laptop, MiFi, and portable printers provided for documentation.
• Documentation systems used include BayCare Forms, BayCare Scheduling, and Ace Timesheet.
• BayCare Online Learning Center modules must be completed prior to start (4-8 hours).
• Dress code and tobacco policy must be followed.
• Photo for badge required prior to start.
• Personal or auto insurance must be current and active during assignment.

Summary

This is a travel contract position for a Physical Therapist Assistant specializing in home health care. The role requires a minimum of 2 years of home health experience, valid PTA licensure, BLS certification, and compliance with all health and background requirements. The position involves self-scheduling home visits with a diverse patient population and requires reliable transportation.
