Required Qualifications

Valid RDN - Registered Dietitian Nutritionist license in Georgia
RDN - Registered Dietitian Nutritionist (2 years of experience)
RDN - Registered Dietitian Nutritionist experience within the last year

Required Health and Compliance Items

Physical exam within 12 months of start
Negative TB test or chest x-ray within 12 months of start, or required documentation for prior positive TB history
TB questionnaire
Hepatitis B surface antibody titer or signed declination
Flu vaccine or declination
Drug screen within 60 days of start
Measles, mumps, rubella, and varicella immunity documentation
Tdap is strongly recommended
Full compliance required before start
