PTA Home Health - Travel/Contract

Location: San Ramon, California
Contract Length: 13 weeks

Job Details

• Setting: Home Health
• Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week
• Hours: 40 hours per week guaranteed (36 hours weekly guaranteed after first 2 weeks)
• Patient Load: 5-6 patients per day, 1:1 ratio; productivity standard is 27 points
• Weekend Work: Optional to pick up patients to meet productivity goals
• On-Call: Not required
• Transportation: Must have own reliable transportation; occasional travel radius up to 50+ miles
• Scrubs: Any color allowed
• Parking: Free
• Mileage Reimbursement: IRS standard

Required Qualifications

• Valid Physical Therapy Assistant license in California
• Minimum 1 year of Physical Therapy Assistant experience, including experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

Compliance & Documentation Requirements

• Government-issued photo ID (Driver’s License/State ID)
• Emergency Contact Form
• I9 Attestation
• Background checks including 7-year work/living history, sex offender registry, and federal exclusions (updated annually)
• Current physical exam within 1 year, cleared to work without restrictions
• Immunizations and titers as applicable:

  • Tetanus/Diphtheria/Tdap (within 10 years or annual declination)
  • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (2 vaccines or positive titer)
  • Flu vaccine or declination required year-round
  • COVID-19 vaccine documentation or declination form required
• Drug screening: 10-panel drug screen within 1 year
• Auto insurance and registration (must be current throughout assignment)
• Professional references: 2 for travel/allied/per diem acute care roles (within 3 years)
• Work history verification: 1 year related employment verification required
• Competency exams and medication/pharmacology tests as applicable

Additional Notes

• The system groups clients to minimize driving and windshield time
• Timely communication and documentation are required; documentation tools will be provided and must be returned
• Locals are accepted for this position

This position is ideal for experienced PTAs with home health expertise who are ready for a travel contract in California and can manage a moderately high patient load with reliable transportation.
