Physical Therapist - Home Health (Travel/Contract)

Location: Lacey, Washington
Employment Type: Travel/Contract
Shift: Day (5x8 hours), 8:00 AM – 4:30 PM
Hours: 40 hours per week


Job Overview

  • Specialty: Home Health Physical Therapy
  • Patient Load: Average 5-7 patients per day
  • Productivity: 6 daily visits or minimum 30 points weekly
  • Case Types: Geriatric patients
  • Territory: Up to 60-mile radius from office, occasionally more based on patient needs
  • Dress Code: Scrubs (any color), no jeans
  • Device Provided: iPad
  • Cell Phone Use: Not allowed
  • Mileage Reimbursed at IRS standard rate
  • Flexibility with territory expected
  • Timely documentation and communication required
  • Locals accepted at travel rate
  • First-time travelers not accepted

Required Qualifications

  • Valid Physical Therapist license in Washington
  • Minimum 1 year of recent experience as a Physical Therapist (within the last year)
  • Basic Life Support (BLS) certification

Contract Length

  • 13 weeks

Background and Compliance Requirements

  • Background checks within 90 days of start, including:
    • SSN Trace
    • County searches (last 7 years)
    • FACIS Level 1
    • Nationwide Sex Offender
    • Motor Vehicle Records/Driver’s Record
    • Education verification (highest degree related to profession)
    • Work history verification (7 years)
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days
  • Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site)
  • State-specific criminal history and employability status checks as applicable
  • Agency background attestation required prior to start

Health and Safety Requirements

  • Physical exam within last 12 months (physician statement)
  • Fit test for N95 mask (no specific model required)
  • Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
  • Annual TB questionnaire required for all candidates
  • Immunizations or positive titers required for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B (series, titer, or declination)
    • Varicella (series, titer, or declination)
  • 9-panel drug screen within last 12 months
  • Seasonal flu vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required

Documentation and Other Requirements

  • Signed Orientation Safety Manual Acknowledgment (updated annually)
  • State license primary source verification within 90 days of start and upon expiration
  • Medication/pharmacology competency exam as applicable
  • Skills checklist required annually
  • Employment application with full 7 years of work history, no gaps
  • Two professional references within 2 years of start (travel, allied, per diem acute care)

Additional Notes

  • Proof of current auto insurance required due to driving responsibilities
  • Candidate must comply with facility’s Acceptable Use Policy prior to start
  • No on-call responsibilities

This position is suited for experienced Home Health Physical Therapists who have prior travel experience and are comfortable managing a flexible patient territory within Washington state.
