Job Opportunity: Medical Surgical LVN / LPN

Contract Length: 13 weeks

Job Details

• Shift: Day shift, 3 shifts per week, 12 hours each (11:00 AM – 11:30 PM)
• Guaranteed hours: 36 hours per week
• Weekend Requirement: Every other weekend (2 weekends per month)
• Mandatory floating within scope to meet facility needs
• Facility type: Correctional health services

Required Qualifications

• Graduate from an accredited Practical Nursing program in Illinois
• Current Illinois Practical Nurse (LPN) license
• BLS (Basic Life Support) certification (AHA only)
• Successful completion of medication administration coursework (Pharmacology) per Illinois Department of Financial and Professional Regulation standards (documentation may be requested)
• Minimum 2 years recent full-time paid clinical experience in acute care or ambulatory care setting (preferred)

Required Certifications & Competencies

• Medication Test/Pharmacology Competency Exam (required for starts on/after 4/14/24; must be completed within 12 months of start)
• Color Blind Testing (required for starts on/after 1/16/24; must be completed within 12 months of start)
• Competency Exam (required for starts on/after 4/14/24; must be completed within 12 months of start)
• BLS certification (AHA only)

Health & Safety Requirements

• Physical exam within 12 months of start, updated annually (signed by PA, NP, DO, or MD)
• Fit mask test within 1 year of start; annual renewal required (3M 1860, 1870+, 9205+, or Halyard Fluidshield N95 masks accepted)
• Tdap vaccine within 10 years or signed declination (declination must be within 12 months of start)
• Negative TB test (QGold or TSPOT) within 12 months of start, updated annually (PPD not accepted after 12/15/2025)
• Immunizations:

  • Measles (Rubeola): 2 MMR vaccines or positive titer (booster if low titer)
  • Mumps: 2 MMR vaccines or positive titer (booster if low titer)
  • Rubella: 2 MMR vaccines or positive titer (booster if low titer)
  • Varicella: 2 vaccines or positive titer (booster if low titer)
  • Hepatitis B series, titer, or signed declination (declination within 1 year)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (10/01 – 5/4)
• Negative 9-panel drug screen within 90 days of start; must be initiated within 48 hours of signed contract; marijuana testing not required

Background & Screening Requirements

• Background check within 12 months covering all locations lived/worked/schooled in past 7 years; must include all aliases and SSN trace
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Illinois Healthcare Worker Registry search required for aides and nursing assistants (not required for LPNs)
• International background check if candidate has lived/worked internationally in past 7 years
• State-specific searches and abuse registry checks are for tracking only and not required by client

Facility-Specific Requirements

• Completion of facility-specific forms for LVNs/LPNs (legal name required, no nicknames; signature and dates required)
• Mandatory floating to other units as needed
• No call requirements
• Return to work (RTO) policy: max 5 days off with no more than 2 consecutive; no RTO on holidays or weekends or day before/after holidays
