Physical Therapist - Colorado (Grand Junction)
Job Type
- Temporary contract-to-hire position
- Onsite work
- Contract length: 15 weeks
- Work schedule: Monday to Friday, 8:00 AM to 5:00 PM (40 hours per week)
Job Description
- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans to maximize patient independence and function
- Regularly reassess treatment effectiveness and patient progress; modify goals or treatment as needed
- Communicate patient needs and progress to treatment team, physicians, patients, and families as applicable
- Consult with medical providers regarding patient evaluation, treatment, progress, and discharge planning
- Recommend and assist in securing special adaptive equipment to improve patient function
- Maintain and submit evaluation, treatment, and billing documentation according to company, facility, and regulatory requirements
- Adhere to company policies, HIPAA, corporate compliance, professional practice standards, and applicable laws
- Supervise Physical Therapist Assistants and Physical Therapy Aides in patient care and related activities per state practice act
- Participate in or conduct staff development programs and meetings; obtain continuing education as required
Required Skills and Experience
- Valid Physical Therapist license in Colorado
- Experience in a Home Health setting
- Experience with Homecare Homebase
Work Location Coverage
- Mesa County and Garfield County areas

- Mileage reimbursement provided for travel within coverage area
