Physical Therapist - Colorado (Grand Junction)

Job Type

Temporary contract-to-hire position
Onsite work
Contract length: 15 weeks
Work schedule: Monday to Friday, 8:00 AM to 5:00 PM (40 hours per week)

Job Description

Perform physical therapy evaluations and treatments based on physician orders/referrals
Complete comprehensive evaluations using appropriate tools and procedures
Review patient records for diagnosis, medical history, precautions, and contraindications
Establish measurable goals and develop treatment plans to maximize patient independence and function
Regularly reassess treatment effectiveness and patient progress; modify goals or treatment as needed
Communicate patient needs and progress to treatment team, physicians, patients, and families as applicable
Consult with medical providers regarding patient evaluation, treatment, progress, and discharge planning
Recommend and assist in securing special adaptive equipment to improve patient function
Maintain and submit evaluation, treatment, and billing documentation according to company, facility, and regulatory requirements
Adhere to company policies, HIPAA, corporate compliance, professional practice standards, and applicable laws
Supervise Physical Therapist Assistants and Physical Therapy Aides in patient care and related activities per state practice act
Participate in or conduct staff development programs and meetings; obtain continuing education as required

Required Skills and Experience

Valid Physical Therapist license in Colorado
Experience in a Home Health setting
Experience with Homecare Homebase

Work Location Coverage

Mesa County and Garfield County areas

Additional Information

Mileage reimbursement provided for travel within coverage area
