Employment Type: Contract
Contract Length: 13 weeks

Float between two clinic locations in Peoria and Surprise, AZ
Clinic schedules:
- Peoria: Monday & Wednesday (10 AM - 7 PM), Friday (8 AM - 5 PM)
- Surprise: Tuesday & Thursday (7 AM - 4 PM)
Patient Age Groups: Adults, Adolescents, Geriatrics
Caseload: 25-35 cases per week, with 10-15 visits per day
EHR System: NextGen
Dress Code: Business Casual (Khakis and polos)

Screening and building caseload
Evaluation/Assessment methods include:
- Fall Risk Assessment
- Gross Motor Assessments
Orthopedic skills with conditions such as:
- Achilles Tendonitis, ACL Tear, Back Pain (Post Operative), Calf Strain
- Carpal Tunnel Syndrome, Disc Herniation, Fractures/Stress Fractures
- Generalized Low Back Pain, Headache, Meniscus Tear, Osteoarthritis, Osteoporosis
- Plantar Fasciitis, Postural Dysfunction, Rotator Cuff (Non-surgical & Post Operative Care)
- SI Joint Dysfunction, Stenosis, Thoracic Outlet Syndrome, TMJ Dysfunction
- Total Joint Replacement
Neurological conditions including concussion, Parkinson’s disease, Vestibular/Vertigo
