Job Summary
The Registered Nurse (RN) provides professional nursing care to a group of patients within a comprehensive rehabilitation center. Responsibilities include patient assessment, development and implementation of nursing and rehab care plans, documentation, and collaboration with the interdisciplinary rehabilitation team. The RN will support patients with physical disabilities and chronic illnesses, providing education and resources to patients and families to promote self-care and wellness.
Key Responsibilities
- Perform initial patient assessments upon admission and develop appropriate nursing and rehab care plans.
- Implement and modify nursing interventions and rehab procedures based on continuous evaluation.
- Maintain accurate and up-to-date documentation of nursing care.
- Coordinate nursing care activities with the interdisciplinary rehabilitation team to achieve patient goals.
- Educate patients and families about disease processes and nursing techniques to support rehabilitation and self-care.
- Participate in rehab rounds and team meetings.
- Ensure patient and employee safety by identifying and addressing hazards.
- Fulfill all departmental competencies related to nursing and rehabilitation care.
Required Qualifications
- Minimum of 3 years of relevant nursing experience.
- Illinois RN license in good standing.
- Current BLS (Basic Life Support) certification.
- Education: Associate of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred.
