Patient Ratios: Approximately half census case load
Patient Population: Inpatient Rehabilitation Facility (IRF)
Common Diagnoses/Treatments: Stroke, Traumatic Brain Injury (TBI), hip fractures, trauma, acute renal failure (ARF), wound care, amputation
Electronic Medical Record (EMR): EPIC

Valid RN license in Colorado
Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Minimum 1 year of case management experience, including acute hospital case management
Case management experience within the last year
Utilization Review experience

Note: No call or float requirements. This is a travel assignment in a Level 1 trauma neuro and trauma rehab unit.
