Required Qualifications
- Valid Pharmacist license in Virginia
- 2 years of Pharmacist experience
- Pharmacist experience within the last year
- Experience in a Hospital setting
- Dose Edge IV experience
- Pyxis experience
- Vigilanz
- Vancomycin AUC dosing experience
- Aminoglycoside dosing
- Renal dosing
- Warfarin experience
- Enoxaparin experience
- Electrolyte protocols
Preferred
- Silver/purple scrubs preferred
Relevant Clinical Experience
- Complex patients including Trauma/Burn/Critical care, peds, cardiac surgery, and ECMO
- Dose Edge IV experience
- Pyxis experience
- Meditech
- Vigilanz
- Vancomycin AUC dosing experience
- Aminoglycoside dosing
- Renal dosing
- Warfarin
- Enoxaparin
- Electrolyte protocols
- Heparin
- DOAC
Unit Responsibilities
- IV verification
- Medication verification
- IV compounding as needed
