Job Details

• Setting: Home Health (Therapy - PT, OT, SLP)
• Average daily census: 78 patients (expected to grow)
• Typical caseload: 5 visits per day
• Common patient diagnoses: Orthopedic/joint replacements, geriatric patients with multiple comorbidities such as CHF and COPD
• EMR system: Netsmart
• Work schedule: Day shifts, 8-hour days, every other weekend (EOW)
• Weekend requirement: Occasional Saturdays for orthopedic patients
• Holidays: Off
• Mileage: Approximately 40 miles per day traveling within Tehama County service area (may decrease with more staff and patient redistribution)
• Staff provided with a laptop for EMR documentation

Additional Information

• PT staff perform Start of Care (SOC) visits, especially for orthopedic patients when nursing is not ordered
• No call or vent responsibilities
• Nurse aides available
• Orientation is not floor-specific
