Job Opportunity: Physical Therapist - Home Health

Location: Sunnyvale, California (Territory includes Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and neighboring cities)
Employment Type: Contract
Contract Length: 13 weeks

Job Description

• Restore function to prevent or minimize disability following disease or injury
• Help clients reach maximum performance/function within their capabilities
• Evaluate and treat patients using medically prescribed physical therapy programs
• May supervise Physical Therapy Assistants
• Conduct initial admission/evaluation visits
• Notify Clinical Team Manager of OASIS scores within established timelines
• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans
• Perform other duties consistent with skills and training aligned with organizational goals and mission

Requirements

• Specialty: Rehabilitation Therapy (Physical Therapist)
• Minimum 1 year of Home Health experience
• Prior travel experience: Not required or not specified
