Work Setting
- Home health role
- Coverage area includes I-25 East Greeley to Castle Rock
- Split between Denver and Loveland offices
- May be asked to help in other territories within the facility’s service area, about a 60-mile radius from the facility address
Required Qualifications
- Valid Physical Therapist license in Colorado
- Experience in a Home Health setting
- Homecare Homebase documentation experience
- Experience with OASIS
Work Expectations
- Guaranteed work week: 40 hours
- Average 4–6 visits per day, depending on visit type
- Productivity expectation: 28–32 points per week
- Overtime must be pre-approved and requires written approval from the Administrator
Cancellation Policy
- 2 weeks cancellation
