Job Description

• Position: MRI Technician
• Work schedule: Four 10-hour night shifts per week (8 PM to 6:30 AM)

Onboarding & Credential Requirements

Personal Health History Requirements (Must be completed before start)

• Fit Test
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (within 1 year for MI, IN, CT, MA; within 5 years for other regions)
• COVID-19 Vaccine: Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
• Influenza Vaccine: Proof or declination/exemption (declination not accepted in CA or IL) for flu season 10/1/2024–4/30/2025
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later); titers not accepted
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Trinity Color Vision test

Professional Requirements (Must be completed before start)

• Trinity Profile: Resume, skills checklist (within 1 year), 2 professional references (required for submission)
• Trinity Skills Checklist: Provided by agency for clinical contractors
• Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement (signed)
• Trinity Conflict of Interest Disclosure Form (signed)
• Criminal Background Check: Federal, nationwide, county (last 7 years), SSN verification, employment verifications (last 7 years), dated within 30 days of start
• Education Verification: Highest academic degree or diploma/GED verified by third party screening
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG Exclusion Check dated within 30 days
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam within last year
• NSO (National Sex Offender) check dated within 30 days, including aliases and statewide check for MI
• System Access Form: Completed and uploaded one week before start
• Code of Conduct Form (signed)
• I9 Form: Completed by agency
• Specialty Specific Competency Testing: Specialty exam completed within last year
• License/Certificate Verification: Must be dated within 30 days of start, with online verification page; for RNs/LPNs use state verification systems (e.g., NURSYS)

Certifications (Must be current and uploaded before start)

• BLS (American Heart Association): Certification with front and back images of card

Summary

This is a 13-week MRI Technician contract position in Columbus, Ohio, working night shifts of 10 hours each, four days per week.
