Licenses & Verification

• Nursys Verification: Must be completed within 30 days prior to start. If any disciplinary actions exist, complete FACIS Questionnaire.
• Current Nursing License: Primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
• Driver's License/State ID: Must be current and match address on file.

Certifications (Must be AHA or equivalent)

• BLS (Basic Life Support)
• ACLS (Advanced Cardiovascular Life Support)
• NRP (Neonatal Resuscitation Program): AHA/American Academy of Pediatrics certified

Exams & Testing (Must be completed prior to start and maintained as required)

• Ascension Pharmacology Exam: Minimum 80% score, updated every 3 years.
• Ascension Specialty Exam: Minimum 80% or national benchmark score, updated every 3 years.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, updated yearly.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
• Ascension Skills Checklist: Clinical skills checklist, updated annually.

Onboarding & Professional Requirements

• Ascension Resume/Profile: Required for submission.
• Two Professional References: Required prior to first assignment.
• Ascension Onboarding Education Attestation: Completion of pre-hire 6-hour training.
• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Biometric Consent & Nova StatStrip Modules: Required pre-hire.
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service >90 days.
• Employment Verification: Past 7 years, updated annually or after break in service >90 days.
• Education Verification: Relevant clinical education verified within 90 days of start; must be rerun if break in service >90 days.
• Confirmation of Assignment: Provided upon initial assignment and updates with extensions.

Health & Immunization Requirements

• MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted.
• Tdap: Valid within 10 years; no declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Optional but recommended; declinations accepted with mask requirement during flu season (Oct 1 - Mar 31).
• Fit Test: Required for all travelers; preferred before start, can be done onsite. Annual update required.
• 2 Step PPD or IGRA: TB testing required with annual questionnaire; if positive, chest X-ray within 1 year required.
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
• 12 Panel Drug Screen: Within 30 days prior to start and annually thereafter; retesting required after break in service >90 days.
