Contract Length

• 14 weeks

Schedule

• Monday to Friday, 8:00 AM to 4:30 PM (Day shift, 5 days/week, 8 hours/day)
• Productivity expectation: 30 visits per week

Required Qualifications

• Valid Physical Therapist license in Maryland
• Experience in a Home Health setting

Required Documentation & Compliance

• Proof of I9 or Attestation
• Government-issued photo ID (Driver’s License required)
• Background check: 7-year criminal history including SS Trace, Counties, Nationwide, Sex Offender, FACIS Level 3; updated annually
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for every contract renewal
• Physical exam within the last 12 months (physician statement)
• N95 Fit Test within 1 year of start, updated annually (can be done onsite)
• Negative TB test or chest X-ray within 1 year, updated annually (2-step PPD, Quantiferon Gold, or T-spot accepted)
• Immunizations or titers required: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella (vaccines, titers, or declination accepted)
• 9-panel drug screen within 1 year of start (instant drug screen acceptable)
• Seasonal flu vaccination or documented exemption required during flu season (Oct 1 - Mar 31)
• COVID-19 vaccination or approved medical/religious exemption with facility-specific form

References & Evaluations

• Two professional references within 2 years required for travel positions
• Performance evaluations required if previously worked with the agency over 1 year
• Internal evaluations required if previously worked over 2 years

Additional Requirements

• Completion of contractor activation form for facility access
• Auto insurance copy required if using personal or auto vehicle for work
• Orientation safety manual acknowledgement and HIPAA compliance documentation
