Physical Therapist - Home Health (Travel/Contract)

Job Overview

  • Position for Physical Therapists providing home health care.
  • Focus on patient-centered care rather than number of patients per day.
  • Minimum 23-24 patient visits per week required to qualify for a 40-hour guarantee.
  • Types of cases include Home Health, Wound Care, and Geriatric Home Health patients.
  • No weekend work required.
  • No on-call requirement (on-call is discretionary but not mandatory).
  • Local candidates within a 50-mile radius are accepted.

Contract Details

  • Contract length: 4 to 6 months (less experienced clinicians must commit to 6 months, non-negotiable).
  • Mileage reimbursed based on IRS standard from first patient to last patient (not to/from home).

Required Qualifications

  • Valid Physical Therapist license in California.
  • Minimum 2 years overall clinical experience as a Physical Therapist.
  • At least 1 year of Physical Therapist experience within the last year.
  • Experience in a Home Health setting.

Compliance and Documentation Requirements

  • Government-issued photo ID (Driver’s License/State ID).
  • Background checks within 1 year, including SS trace, county searches, nationwide sex offender registry, and annual federal exclusions lists.
  • Emergency contact form required prior to start.
  • Physical exam within 6 months signed by MD/DO, including statement of no active communicable/infectious diseases.
  • Immunizations and health screenings required:
    • Tetanus/Diphtheria/Tdap (within 10 years or medical/religious exemption).
    • Negative TB test or chest X-ray within 1 year.
    • Measles, Mumps, Rubella proof of immunity or vaccination as per birth year guidelines.
    • Hepatitis B titer, declination, or vaccine series.
    • Varicella immunity or vaccination history.
    • Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31).
    • COVID-19 vaccination or approved medical/religious exemption.
  • Drug screening: 9-panel drug screen within 1 year of start (instant drug screen acceptable).
  • Auto insurance card required.
  • All required documents must be submitted and approved before starting patient care; compliance processing may take 2-3 weeks.
  • References:
    • 2 professional references for travel, allied, or per diem acute care.
    • 1 professional reference for per diem non-acute care.
