Licensed Practical Nurse (LPN) - Travel/Contract

Location: Montrose, New York
Contract Length: 26 weeks

Job Summary

Provide nursing care primarily to the geriatric population in a long-term care setting. Responsibilities include assisting residents with treatment and rehabilitation activities, maintaining supplies, ensuring cleanliness, and supporting facility operations.

Required Qualifications

• Current license to practice as a Licensed Practical Nurse (LPN) in New York State.
• Minimum 1 year of post-licensure clinical nursing experience (preferred).
• Experience working with the geriatric population in long-term care (preferred).
• Experience serving the Veteran population (preferred).

Key Responsibilities

• Communicate with residents, respond to their concerns, and encourage participation in treatment and therapeutic activities.
• Inventory and order supplies to maintain appropriate stock levels.
• Change bed linens and maintain cleanliness to ensure a sanitary environment.
• Assist with cleaning tasks as needed to maintain facility standards.
• May assist with washing and dressing of deceased individuals following standard procedures.
• Attend mandatory educational programs.
• Be knowledgeable in fire and disaster preparedness procedures.
• Adhere to established attendance policies and procedures.

Certifications & Competencies Required

• Basic Life Support (BLS) certification (American Heart Association or American Red Cross).
• Advanced Cardiac Life Support (ACLS) certification (American Heart Association or American Red Cross).
• Pediatric Advanced Life Support (PALS) certification (American Heart Association or American Red Cross).
• Competency exams as required for LPN candidates.

Background & Health Requirements

• Criminal background check including fingerprinting (completed onsite except for certain facilities).
• Physical exam within 1 year of start date.
• Negative TB test or chest X-ray within 12 months prior to start.
• Annual TB questionnaire completion.
• Vaccination documentation or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (or declination)
  • Varicella
• Drug testing (9 panel) within 1 year of start.
• Seasonal flu vaccination or declination for 2025-26.
• COVID-19 vaccine booster preferred but not required.

Additional Requirements

• Two professional references required.
• Compliance with state-specific registry and enrollment documentation as applicable.

• This is a travel/contract position with a 26-week duration.
