Physical Therapist - Inpatient Rehab (Travel/Contract)
Location: San Leandro, California
Contract Length: 13 weeks
Job Details
- Work schedule: Monday to Friday, 8:00 AM to 4:30 PM (5x8 hour shifts)
- Rotate twice a month to cover Saturdays; when working Saturday, flex off one weekday
- No on-call duties
- Work setting: Acute Rehab Unit (ARU)
- Collaborate as part of an interdisciplinary team
- Previous experience in Acute Rehab Unit or Skilled Nursing Facility (SNF) is desirable
- No overtime approved unless a written exception is provided
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 2 years of experience as a Physical Therapist, including experience within the last year
- Valid California Occupational Therapist (OT) license
- Basic Life Support (BLS) certification
Required Documentation & Compliance
- Valid Driver’s License or Passport (photo ID, color, clearly visible)
- Background check within 90 days prior to start, including multiple criminal and sanction searches
- Education verification confirming minimum required education completed
- Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform job duties
- Fit mask test within 90 days of start (specific approved masks required)
- Immunizations and health screenings:
- Tetanus/Diphtheria/Tdap (within 10 years or signed declination)
- Negative TB test or chest X-ray updated annually
- Proof of MMR (Measles, Mumps, Rubella) vaccinations or positive IgG titers
- Hepatitis B positive titer or completed vaccine series (declination allowed)
- Varicella vaccinations or positive IgG titer
- Seasonal flu vaccination or declination on file by November 6 (masking required during flu season)
- Drug screening: Oral saliva drug test within 30 days of start (rapid tests not accepted)
- Color blind exam within 90 days of start (Ishihara test accepted)
- Employment history verification for the past 3 years and current resume including last 7 years of employment
- References per agency requirements (not facility requirement)
- Completion of facility-specific competencies and new employee orientation modules on first day of work
Additional Notes
- This is a travel/contract position covering a leave of absence until late October
- Badge photo and facility-specific forms required prior to start
- Competencies must be completed by 11:59 PM PST on the first day of the contract; failure to complete within 14 days results in immediate termination
- No prior travel experience explicitly required for this position
