Job Opportunity: Home Health Registered Nurse

Location: Lebanon, Pennsylvania (Coverage includes Lancaster, Lebanon, Eastern York Counties)
Employment Type: Travel/Contract
Contract Length: 13 weeks


Schedule & Work Hours

  • 40 hours per week, Monday to Friday, 8:00 AM – 4:30 PM
  • Every 4th weekend rotation required
  • Holidays as needed
  • Floating across county lines possible if patient care requires

Required Qualifications

  • Valid Registered Nurse (RN) license in Pennsylvania
  • Minimum 2 years of Home Health nursing experience, with at least some experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
  • Experience with OASIS documentation

Patient Population & Work Environment

  • May care for COVID-19 patients
  • Tobacco and fragrance-free facility environment

Onboarding & Compliance Requirements

Background & Screening

  • 7-year criminal background check required within 30 days of start, updated annually
  • Pennsylvania State Police PATCH search required, must be finalized prior to start (no pending results allowed)
  • Pennsylvania Child Abuse (Childline) search required, must be completed 3 business days prior to start, renewed every 5 years
  • FBI fingerprinting required within 1 year of start; candidates may start with results pending but must have results on file within 45 days or will be removed from assignment
  • EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
  • Education verification of highest degree required
  • Drug testing: Negative 9-panel drug screen within 1 year of start (requirement applies to agency only, not facility)

Health & Immunization

  • Physical exam within 90 days of start, signed by MD, DO, PA, or NPT
  • Fit mask testing required within 1 year of start and updated annually; specific approved mask types mandated
  • Immunizations/vaccinations required with documentation including:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Negative TB test (QGold or TSpot) within 90 days of start; PPD not accepted
    • Measles (Rubeola), Mumps, Rubella: 2 vaccines or positive titer required; boosters required if titers negative or equivocal
    • Hepatitis B: Positive titer or full vaccine series (3 doses) or declination form required
    • Varicella: 2 vaccines or positive titer required; boosters required if titers negative or equivocal
    • Seasonal Flu vaccine required during flu season or valid medical/religious exemption (declinations not accepted)
    • COVID-19 vaccine or valid medical/religious exemption/declination accepted

Certifications & Competencies

  • Acknowledgement of Pass/Fail Testing required within 48 hours of signed contract (except OR and Home Health)
  • Medication/Pharmacology competency exam as applicable
  • Dysrhythmia/EKG competency exam required for all RNs except OR and Home Health (pass/fail exam with remediation process)
  • Additional certifications (ACLS, PALS, NRP, TNCC, NIH Stroke Certification) may be required depending on unit/job description

Documentation & Facility Requirements

  • Clear, passport-style photo for badge (no masks, hats, sunglasses; religious headwear allowed)
  • Copy of auto insurance required for candidates working in home care setting
  • Completion and signing of facility-specific documents including:
    • Medical Statement Non-Employee Ability to Work form
    • Commitment to Health & Wellness
    • Code of Conduct
    • Confidentiality Agreement
    • Non-Employee Affidavit
    • I9 Attestation and Documentation Attestation (completed by agency)
    • Residency form for home care candidates
    • Hepatitis B declination form if applicable
    • Safety and Security Vehicle Registration (for home care)
    • Review of facility policies such as "Wellspan Way" description

Orientation & Training

  • New Hire Orientation required, to be completed by end of first day
  • Learning Management System (LMS) modules assigned and must be completed on PC/laptop (not mobile) prior to start
  • Multi-factor authentication and user account setup required for LMS access

References & History

  • Two professional references required within 3 years of start
  • Most recent resume or employment application required
  • Annual skills checklist required

Additional Notes

  • Return to Office (RTO) maximum 7 days
  • No call requirements
  • Floating may involve crossing county lines
  • Tobacco and fragrance-free environment; candidates must be notified prior to submission
