Occupational Therapist - Home Health (Travel/Contract)

Location: Lacey, Washington
Contract Length: 13 weeks


Job Details

  • Schedule: Monday to Friday, 8:00 AM to 5:00 PM (Day shift)
  • Guaranteed 40 hours per week
  • Average caseload: 5-7 patients per day (6 visits daily or minimum 30 points weekly)
  • Patient population: Geriatric
  • Territory: Up to 60-mile radius from the office/hub
  • Dress code: Scrubs (any color), no jeans
  • Device provided: iPad (cell phone use not allowed)
  • Mileage reimbursed at IRS standard rate
  • On-call: No
  • Weekend work: Not required, but some flexibility to work weekends

Required Qualifications

  • Valid Occupational Therapist license in Washington
  • Minimum 1 year of Occupational Therapist experience within the last year
  • Basic Life Support (BLS) certification
  • Experience with OASIS
  • Experience with Homecare Homebase

Compliance & Background Requirements

  • Background checks within 90 days of start, including:
    • SSN Trace
    • County searches (last 7 years)
    • FACIS Level 1
    • Nationwide Sex Offender
    • Motor Vehicle Records/Driver’s Record
    • Education verification (highest degree related to profession)
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
  • Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site)
  • State-specific criminal history and employability status checks as applicable
  • Employment history verification for 7 years required
  • Two professional references within 2 years of start

Health & Safety Requirements

  • Physical exam within the last 12 months (not required annually)
  • Fit test for N95 mask (no specific model required)
  • Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
  • Annual TB questionnaire required
  • Immunizations or titers for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Seasonal flu vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required
  • 9-panel drug screen within 12 months of start (instant acceptable)

Additional Requirements

  • Proof of current auto insurance required due to driving responsibilities
  • Completion of facility-specific Acceptable Use Policy prior to start
  • Signed acknowledgment of Orientation Safety Manual (updated annually)
  • Annual skills checklist and competency exams as applicable

Notes

  • Local candidates are accepted and paid at travel rates
  • No on-call or weekend requirements, but some weekend flexibility is available
  • Cell phone use is not permitted during work; iPad provided for documentation and communication
