Job Opportunity: Home Health Registered Nurse

Location: Portland, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Day shift, 8 hours per day (5x8 hours), 8:00 AM to 4:30 PM
  • Patient Ratio: 1:1 (may visit up to 6 patients per day)
  • Productivity Standard: 30 productivity points per day
  • Weekend Requirement: None
  • On-Call: Not required
  • Floating: Not required
  • Block Schedule: Not accommodated
  • Locals: Accepted within 50-75 mile radius
  • Start Date: ASAP
  • Guaranteed Hours: Facility may call off up to one shift every two weeks
  • Transportation: Must have own transportation; daily travel radius up to 60 miles (60-90 minutes total drive time)

Required Qualifications

  • Valid RN license in Oregon
  • Minimum 1 year of Home Health nursing experience within the last year
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Basic Life Support (BLS) certification issued by American Heart Association
  • Experience in a Home Health setting
  • Experience with OASIS

Work Environment

  • Facility Type: Home Health
  • Attire: Business casual; scrubs allowed but no specific color required; no jeans
  • Charting Systems: EPIC Remote Client (laptop), Rover (phone app), and OASIS
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Compliance and Documentation Requirements

  • Background Check: Must be within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide checks, motor vehicle record, and others as applicable
  • Education Verification: Required for nursing degree; international education requires diploma/transcripts or CES report
  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
  • Drug Testing: 9-panel drug screen within 12 months of start
  • Immunizations and Health Requirements:
    • Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
    • Negative TB test or chest X-ray within 12 months (annual updates required)
    • Measles, Mumps, Rubella (2 MMR vaccines or positive titer; declination accepted)
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer (declination accepted)
    • Seasonal Flu vaccine during flu season (declination accepted)
    • COVID-19 vaccination record preferred; declinations not required but unvaccinated must follow masking policies
  • Respiratory Safety Clearance: Required annually for respirator use (travel only)
  • Fit Mask Testing: Required annually for travel candidates (onsite or valid record accepted)

Additional Requirements

  • References: Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting
  • Employment Application: Must include at least last 5 years of employment with month/year format
  • Skills Checklist: Required and must be updated annually
  • NURSYS Verification: Required for all RNs
  • Demographic Attestation: Required for nurses working with out-of-state compact licenses in Washington
  • State-Specific Searches: Washington WATCH search required for WA and border placements; Oregon Abuse Neglect search as applicable
  • Photo for Badge: Passport-style photo with neutral background required

Notes

  • First-time travel nurses are accepted
  • Pending licenses accepted only if both OR and WA licenses are held
  • Facility cannot accommodate 4x10 or 3x12 shifts
  • No weekend or on-call requirements
  • No floating between units
  • Must confirm any Return to Office (RTO) restrictions with hiring manager

This position is ideal for a Registered Nurse with home health experience who holds both Oregon and Washington licenses and is comfortable with travel within the Portland area and surrounding regions.
