Job Overview

Required Qualifications

• Valid RN license in Indiana
• Associate of Science in Nursing / Associate Degree in Nursing
• Operating Room experience within the last year
• 1 year of Operating Room experience

Compliance and Onboarding

• 7-year background check must include State Medicaid, SSN verification, Education Verification, and National Sex Offender Registry
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days
• Orientation safety manual acknowledgement required
• Competency exam and medication/pharmacology competency exam required as applicable
• Clear passport-style photo required for badge
• Attestation must be completed and uploaded within 48 hours of candidate signing
• Parking permit application required for onsite parking

Health Requirements

• Physical exam required within 30 days of start
• Negative TB test or chest X-ray required within 1 year of start
• Measles, mumps, rubella, hepatitis B, and varicella immunity or vaccination documentation required
• 9-panel drug screen required within 30 days of start
• Flu vaccination or medical/religious exemption required during flu season
• Respiratory safety clearance required
• N95 fit testing completed during orientation
