Required Qualifications

  • Valid California Pharmacist license
  • Doctor of Pharmacy degree
  • 3 years of pharmacist experience
  • Pharmacist experience within the last year
  • Experience in a hospital setting
  • Experience in an ambulatory care setting

Department-Specific Requirements

  • Emergency Medicine or Emergency Medicine Mid-shift Pharmacist Specialist: 1 year of Emergency Department pharmacy experience and/or 1 year of hospital-based clinical pharmacist experience in critical care or equivalent setting
  • Infectious Disease Specialist: 1 year of hospital-based clinical pharmacist experience
  • Anemia Management Specialist: 1 year of hospital-based clinical pharmacist experience
  • Home Infusion: Pharm.D. and 1 year of home infusion pharmacy experience and/or 1 year of hospital-based clinical pharmacist experience, or 5+ years of home infusion pharmacy experience
  • Critical Care Medicine Pharmacist Specialist: 1 year of hospital-based clinical pharmacist experience and/or 1 year of hospital-based clinical pharmacist experience in critical care
  • Ambulatory Care Pharmacist Specialist: 1 year of Ambulatory Care pharmacy experience and/or completion of a pharmacy residency program in Ambulatory Care or General Practice with strong Ambulatory Care experience and certification, or eligibility to sit for a certification exam
  • Family Medicine Pharmacist Specialist: completion of a pharmacy residency program in General Practice with strong Ambulatory Care experience and certification, or eligibility to sit for a certification exam
  • Outpatient Oncology Pharmacist Specialist: completion of PGY-1 residency or at least 2 years of acute inpatient hospital pharmacist experience

Key Responsibilities

  • Reviews, validates, and dispenses medications and related orders in inpatient and outpatient settings
  • Consults with other healthcare providers to optimize therapy and medication safety
  • Monitors patient therapies and documents interventions and clinical charting
  • Provides medication education, consultation, and in-service training
  • Supports transitions of care, medication reconciliation, and care coordination when assigned
  • Participates in rounds, formulary work, policy/procedure development, and medication use evaluations when applicable

Additional Notes

  • Uses the EMR for documentation in the outpatient oncology role
  • May be assigned to specialty areas including emergency medicine, oncology, infectious disease, anemia management, pain management, critical care, NICU/pediatrics, formulary management, compounding, ambulatory care, family medicine, or home infusion
