Job Details
- Shift: Day shift; shifts rotate between closer and opener (5:00 pm – 5:30 am)
- On-call: Weeknight and weekend on call
- Patient Ratio: 1:1

- Fem-Pop/Fem-Tib-Peroneal Bypass
- Angiogram
- Pulmonary decortication
- Ablation
Requirements
- Experience: Minimum 2 years of nursing experience
- Specialty Experience: PreOp/PostOp required
- Electronic Medical Record: Epic experience required
- Certifications:
- BLS (Required)
- ACLS (Preferred)
- Preferred Qualifications:
- Level 1 trauma/outpatient experience
- PACU experience
- Previous charge nurse experience
Certifications & Competencies
- Basic Life Support (BLS): AHA Instructor/Provider or RQI acceptable
- Advanced Cardiac Life Support (ACLS): AHA Instructor/Provider or RQI acceptable (preferred)
- Medication Test/Pharmacology Competency Exam: Required prior to start
- Unit-Specific Competency Exam: Required
- Dysrhythmia/EKG Competency Exam: Required within 1 year of start and updated annually (with remediation process if needed)
Background & Compliance
- 7-year criminal background check within 90 days of start, including SS trace, counties lived/worked, nationwide check, sex offender registry, and motor vehicle record if applicable
- FBI and BCI fingerprinting required within 1 year of start (Ohio-specific code 3701.881)
- Monthly EPLS/GSA/SAM and HHS/OIG monitoring
- Education accreditation verification from an accredited nursing program (CCNE, ACEN, NLN CNEA, or CASN)
- Transcript/Diploma verification required (official transcripts only)
- Pending background check attestation required if fingerprint results are pending
Health & Immunization Requirements
- Physical exam within 1 year of start confirming good health and free of contagious disease
- Tetanus/Diphtheria/Tdap (adult dose after age 11/12)
- Negative TB test (PPD, QGold, or TSPOT) within 1 year or chest x-ray if positive
- TB questionnaire required in lieu of annual TB test renewal
- MMR (Rubeola, Mumps, Rubella): 2 vaccines or positive titer; boosters if low titer
- Hepatitis B: Titer or 3-vaccine series or signed declination
- Varicella: 2 vaccines or positive titer; medical exemption requires documentation
- Negative 10-panel drug screen within 3 months of start (no pending tests allowed)
- Seasonal flu vaccination or declination during flu season (10/1 - 3/31); mask required if declining
Additional Requirements
- 2 professional references required (at least 1 from a manager/director or formal leadership)
- Employment application and skills checklist required
- Annual acknowledgment of dress code and hand hygiene policy
- Annual fraud, waste, and abuse training certificate required
- Annual review and acknowledgment of ethics and compliance policies
- Orientation safety manual acknowledgment required annually
