Health and Safety Requirements

• Physical Exam (annual renewal required)
• Fit Mask Test (approved facility mask, annual renewal, must be completed prior to arrival)
• Tetanus/Diphtheria/Tdap vaccination proof
• Negative TB test or Chest X-Ray (within 1 year; if positive, chest X-ray within 5 years)
• TB Questionnaire (annual renewal)
• Rubeola (Measles) immunity proof (titer or 2 vaccines)
• Mumps immunity proof (titer or 2 vaccines)
• Rubella immunity proof (titer or 2 vaccines)
• Hepatitis B immunity or vaccine series or declination (facility-specific declination required if applicable)
• Varicella immunity proof (titer or 2 vaccines)
• Drug Testing: Negative 10-panel within 30 days of start (no instant tests)
• Seasonal Flu Vaccination or Medical Exemption (required during flu season; declinations not accepted)
• COVID-19 Vaccination: Required for all patient-facing specialties (initial series, J&J dose, or seasonal/bi-valent vaccine accepted; medical exemptions accepted with doctor’s letter; religious exemptions not accepted)
