Health and Safety Requirements
- Physical Exam (annual renewal required)
- Fit Mask Test (approved facility mask, annual renewal, must be completed prior to arrival)
- Tetanus/Diphtheria/Tdap vaccination proof
- Negative TB test or Chest X-Ray (within 1 year; if positive, chest X-ray within 5 years)
- TB Questionnaire (annual renewal)
- Rubeola (Measles) immunity proof (titer or 2 vaccines)
- Mumps immunity proof (titer or 2 vaccines)
- Rubella immunity proof (titer or 2 vaccines)
- Hepatitis B immunity or vaccine series or declination (facility-specific declination required if applicable)
- Varicella immunity proof (titer or 2 vaccines)
- Drug Testing: Negative 10-panel within 30 days of start (no instant tests)
- Seasonal Flu Vaccination or Medical Exemption (required during flu season; declinations not accepted)
- COVID-19 Vaccination: Required for all patient-facing specialties (initial series, J&J dose, or seasonal/bi-valent vaccine accepted; medical exemptions accepted with doctor’s letter; religious exemptions not accepted)

- Background checks including SS Trace and County searches for last 7 years
- Sex Offender Search within 30 days of start
