Position Details

• Specialty: Occupational Therapy (OT) Outpatient with Lymphedema experience required
• Minimum Years of Experience: 1 year
• First-time travelers accepted
• Shift Options:

  • 4x10 hour days (8:00 AM - 4:30 PM)
  • Open to 5x8 hour days as well
• Weekend Requirement: Rotating every 6 weeks
• On-call: Not required
• Floating: May float within scope to meet facility needs
• Block Schedule: Possible, to be discussed during interview
• Locals accepted with no radius restrictions
• Return to Office (RTO) Restrictions: Ideally no more than 5 days

Required Certifications & Licenses

Experience Requirements

• Minimum 1 year of experience in OT Outpatient with Lymphedema
• Prior travel experience: Not explicitly required

Background & Compliance Requirements

• Background Check within 30 days of start covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
• Fingerprinting: Illinois Healthcare Worker Registry Search required for clinical candidates without IL state license; must be completed in Illinois 3 business days prior to start
• Education Verification: Initial degree verification required
• Competency Exam: Required for all clinical and allied candidates
• References: 2 professional references required for direct travel allied candidates

Health & Safety Requirements

• Physical Exam: Within 1 year of start, must indicate good physical and mental health, free of communicable disease, and ability to perform job functions without restrictions
• Fit Mask Test: Required for new contracts (specific mask types accepted)
• Negative TB Test or Chest X-Ray: Within 1 year of start
• TB Questionnaire: Required annually
• Immunizations:

  • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines, positive titer, or signed declination)
  • Hepatitis B vaccine series, positive titer, or signed declination
  • Varicella (2 vaccines, positive titer, or signed declination)
• Drug Testing: 9-panel drug screen within 30 days of start
• Flu Vaccination or Medical/Religious Exemption required annually by 11/30
