Location: Dothan, Alabama

Contract Length: 16 weeks

Job Details

• Shift: Nights, 7 days on / 7 days off (2000-0630)
• Weekend/Call Requirements: Every other weekend as part of 7 days; no call required
• Desired Start Date: December 1st

Required Experience

• Minimum 2 years hospital experience in Pharmacy
• First-time travelers are welcome (no prior travel experience required)

Required Certifications and Licenses

• Registered Pharmacist (RPh) license in Alabama
• IV Certification
• Alabama State Pharmacist License

• Pyxis

Required Qualifications and Compliance

• Must provide proof of work eligibility (I-9, E-Verify)
• Background checks including 7-year criminal history, education verification, EPLS/GSA/SAM, HHS/OIG checks
• Drug testing: 10-panel within 90 days of start, updated annually (instant/rapid tests not accepted)
• Physical exam attesting ability to meet job demands, updated annually
• Fit mask test within 12 months prior to start (specific approved mask models)
• Immunizations and health documents required:

  • Negative TB test or chest X-ray within 1 year
  • Tetanus/Diphtheria/Tdap (only if working in Pediatrics)
  • Measles, Mumps, Rubella, Varicella titers or vaccinations (booster or declination required if low titer)
  • Hepatitis B titer, vaccine series, or declination
  • Seasonal flu vaccination required for placements starting on or after 9/15 (no declinations except approved medical/religious exemptions)
  • COVID vaccine is voluntary

Orientation and Facility Requirements

• Completion of facility-specific modules prior to orientation (no time to complete during orientation)
• Photo for badge required within 72 hours of signing (passport-style, no nose rings)
• Preferred name for badge must be confirmed
• Shirley’s Law APS Clearance Form and results must be completed and on file prior to orientation (time sensitive)

Competency Exams and Training

• Medication Test/Pharmacology Competency Exam required
• Core Exams I-III from Prophecy highly preferred (HIPAA attestation required)
• ACLS and BLS certifications recommended (per facility standards)
• NIH Stroke Certification not required for Pharmacy

References and Documentation

• Proof of 2 clinical references within the last 12 months (may extend to 18 months if necessary)
• Complete employment application/resume required
• Skills checklist updated annually (as applicable)
