Physical Therapist - Outpatient (Travel/Contract)

Location: Alameda, California
Contract Length: 13 weeks

Job Details

• Treat 8-9 patients per day with one-on-one physical therapy sessions.
• Point of service documentation required.
• Possible float assignment (to be discussed during interview).
• No weekend shifts.
• Shift: Day shift, 8:30 AM to 5:00 PM (5 days a week, 40 hours/week).

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of Physical Therapist experience, with experience within the last year.
• Basic Life Support (BLS) certification.

Compliance and Documentation Requirements

• Valid photo ID (Driver’s License or Passport) with clear, color photo.
• Background check within 90 days prior to start, including multiple criminal and sanction searches.
• Education verification confirming minimum required education completed.
• Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform job duties.
• Fit mask test within 90 days of start with approved mask types; PAPR accepted if fit test failed (approval required).
• Immunizations and health screenings required:

  • Tetanus/Diphtheria/Tdap (within 10 years or signed declination).
  • Negative TB test or chest X-ray updated annually.
  • Proof of immunity or vaccination for Measles, Mumps, Rubella (MMR), Varicella, and Hepatitis B (positive titer or signed declination).
  • Seasonal flu vaccination or signed declination required annually.
• Drug screening (oral saliva test) within 30 days of start; rapid tests not accepted.
• Color blindness exam within 90 days of start (Ishihara test accepted).
• Employment history verification for the past 3 years and current resume covering last 7 years.
• References per agency requirements (not facility requirement).

Certifications and Competencies

• Basic Life Support (BLS) certification (AHA only).
• Orientation Safety Manual acknowledgment required.
• Competency exams and medication/pharmacology competency as applicable.
• Facility-specific competencies and new employee orientation modules must be completed by the end of the first day of the contract. Failure to complete within 14 days results in immediate termination.

Additional Notes

• No Occupational Therapy (OT) approved unless a written exception is provided.
• Return to Office (RTO) details will be discussed prior to contract signing; no changes accepted after contract is signed.
• Masking required during flu season (Nov 1 – Mar 31) regardless of vaccination status.
