Position Details

• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (40 hours/week)
• Patient Ratio: 1:1 patient to therapist ratio

Job Description

• Provide outpatient physical therapy assistance with a focus on orthopedic populations, young athletes, post-operative patients, individuals with debility/falls, and balance issues.
• Must demonstrate experience and competency in these patient populations.

Required Qualifications

• Valid Physical Therapy Assistant license in Oklahoma.
• Minimum of 1 year of experience as a Physical Therapy Assistant.
• Physical Therapy Assistant experience within the last year.

Required Documentation & Compliance

• Government-issued photo ID must be carried during work hours.
• Proof of E-Verify or attestation required.
• Background checks including a 7-year criminal history, sex offender registry, FACIS Level 3, and education verification as applicable.
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated with contract renewals.
• Transcript or diploma may be used as a placeholder if education verification is pending.
• Drug screening: 9-panel or 12-panel drug screen within required timeframes.
• Physical exam within the last 12 months (not required annually).
• Fit mask (N95) fit test within the last year; annual updates required.
• Immunizations and titers required or declinations accepted for:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Seasonal flu vaccination or approved exemption required during flu season (October 1 – March 31).
• References: 2 professional references within 2 years required for travel positions.
• Completion of employment application/resume.
• Annual competency exams and skills checklists as applicable.
• Confidentiality and non-disclosure agreement, employee health survey, and facility-specific forms must be completed prior to start.

Additional Information

• No special certifications required beyond PTA licensure.
• No on-call or weekend shifts.
• No restrictions on time off.
• Orientation includes safety manual acknowledgement and HIPAA compliance.
• Candidates may complete fit testing on site during orientation if needed.
• PAPR/CAPR respiratory options are not available at this facility.
