Job Description

Seeking a Physical Therapist Assistant (PTA) with home health experience to provide skilled therapy services in patients' homes. The caseload includes patients with medical/surgical, cardiac, orthopedic, neurological, respiratory, and post-surgical conditions.

Key Responsibilities:

• Perform patient assessments
• Manage cardiopulmonary/circulatory care
• Document patient care accurately
• Use and manage equipment and devices such as gait belts, walkers, lifts, collars, supplemental oxygen, prosthetics, wheelchairs
• Develop and implement home exercise programs
• Educate patients and families on medications, safety, injury prevention, and signs/symptoms of abuse (physical, sexual, psychological)
• Provide skilled home care visits (6-8 visits per day)
• Work 1-2 weekends per contract

Patient Population Includes:

• Med/surg, cardiac, ortho (hips/knees/shoulders)
• Post-surgical patients
• COPD, CHF, pneumonia
• Stroke (CVA), Parkinson’s, trauma, respiratory patients
• Amputees (AKA, BKA)
• Patients with weakness, falls, failure to thrive, neurological conditions
• Activities of Daily Living (ADLs) support

Required Qualifications

• Valid Physical Therapist Assistant license in Florida
• Minimum 1 year of experience as a Physical Therapist Assistant, including experience within the last year
• Minimum 1 year of experience in a Home Health physical therapy setting
• Basic Life Support (BLS) certification

Compliance & Documentation Requirements

• Background checks including education verification, criminal history, and federal exclusions
• Florida AHCA Level II fingerprint clearance (must be current prior to start)
• Physical exam within 6 months of start
• Negative TB test or chest X-ray within 10 weeks of start (or TB questionnaire if applicable)
• Immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B
• Drug screening (standard 9-panel within 30 days)
• Annual flu vaccination or declination with mask requirement
• COVID-19 vaccination or approved exemption
• Respirator medical clearance if respirator use is required
• Two professional references from the past 2 years
• Skills checklist and employment application documentation
• Photo for badge (clear, professional, solid background)
• Personal or auto insurance (must be current during assignment)
• Completion of required online training modules prior to start (time sensitive)

Additional Information

• Laptop, MiFi, and portable printers provided for documentation
• Documentation systems and scheduling software training required
• Minimum daily productivity points required (6 points minimum, may increase)
• Candidates must complete all required competencies and certifications as applicable
