Job Description

• Provide physical therapy assistance in a home health setting covering multiple areas including Fremont, Castro Valley, Hayward, Oakland, Berkeley, and surrounding cities.
• Work schedule: 8-hour day shifts.
• Patient ratio: 1:1, with up to 5-6 patients seen per day.
• Guaranteed 36 hours weekly (note: guaranteed hours not applied during first 2 weeks, holidays, or time off).
• Weekend work is optional to meet productivity goals.
• No on-call required.
• Local candidates accepted.
• Own transportation required; travel radius may occasionally exceed 50 miles.
• Timely communication and documentation are essential; documentation tools will be provided.

Required Qualifications

• Valid Physical Therapy Assistant license in California.
• Minimum 1 year of Physical Therapy Assistant experience, including experience within the last year.
• Experience in a Home Health setting.
• Current Basic Life Support (BLS) certification.
• Must have reliable personal transportation.

Compliance & Documentation Requirements

Candidates must complete and maintain the following prior to and during the assignment:

• Government-issued photo ID (Driver’s License/State ID).
• Emergency contact form.
• Background checks including 7-year history and annual federal/state screenings.
• Certifications: BLS (required), ACLS and PALS preferred.
• State license verification.
• Competency exams as applicable.
• Auto insurance and registration (must be current throughout assignment).
• Physical exam within 1 year confirming ability to work without restrictions.
• Immunizations and titers: Tdap, MMR (Measles, Mumps, Rubella), Hepatitis B, Varicella.
• Negative TB test or chest X-ray within 1 year.
• Drug screening (10-panel) within 1 year.
• Seasonal flu vaccination or declination.
• COVID-19 vaccination documentation or facility-specific declination form.
• Professional references and work history verification related to the profession.

Additional Notes

• First-time travelers are not accepted for this assignment.
• The system attempts to group clients to minimize driving time.
• Documentation tools must be returned at assignment end.
