Registered Nurse - Home Health (Georgia)

Job Details

• Location: La Fayette, Georgia (covering Walker and Chattooga Counties)
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
• Average Patients per Day: 5-8
• Weekly Travel: Approximately 250 miles (up to 75 miles radius from home clinic)
• Work Setting: Patient homes and occasional office meetings
• EMR Systems: HomeCare HomeBase (HCHB), OASIS, Outlook
• Dress Code: Scrubs

Job Description

• Coordinate and provide home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
• Manage various case types including Start of Care, Wound Care, and Follow-up Visits.
• Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
• No guaranteed hours; if patient census is low, may be floated to a nearby clinic to pick up hours.
• Weekend and on-call requirements vary; discuss during interview.

Required Qualifications

• License: Valid Registered Nurse (RN) license for Georgia (license must be in hand; pending licenses not accepted).
• Experience: Minimum 1 full year of recent Adult Home Health or Hospice experience (first-time travelers not accepted).
• Certifications: Current CPR and BLS certification.
• Proficiency with HomeCare HomeBase within the last 6 months; if not, retraining required.
• RNs must be proficient in OASIS documentation.
• Must have reliable transportation and current driver’s license.
• Ability to travel up to 75 miles from home clinic.

Additional Requirements

• Must complete various background checks, including seven years of history, state-specific searches, and drug screening.
• Required health documents include:

  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination series or declination.
• Must complete compliance and policy acknowledgments prior to start, including confidentiality, HIPAA, and home health core policies.
• Required to have personal or auto insurance policy under candidate’s name or list candidate as a driver.
• References: Two professional references required.
• Work history verification for at least the last two employers or seven years.

Notes

• Local candidates are accepted at the same rate.
• No on-call duties for this position.
• Some positions may follow a 4x10 schedule (Friday-Monday), but most are standard 5x8.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed).
