Registered Nurse - Home Health (Travel/Contract)

Location: Eugene, Oregon
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40


Job Overview

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
  • Average patient load: 5-8 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day.
  • Types of visits include Start of Care, Wound Care, and Follow-up Visits.
  • Must be willing to travel up to 75 miles from the home clinic; typical weekly travel is approximately 100 miles.
  • Use of electronic medical records systems: HomeCare HomeBase (HCHB) and Oasis (proficiency required).

Required Qualifications

  • Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience (depending on specialty).
  • Recent specialty experience required; first-time travelers are not accepted.
  • Current, valid Registered Nurse (RN) license for Oregon (license must be in hand; pending licenses are not accepted).
  • CPR and Basic Life Support (BLS) certification required (AHA, National CPR Foundation, or ARC accepted).
  • Reliable transportation, current driver’s license, and agency-required liability insurance mandatory.
  • Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last use was over 6 months ago.
  • RNs must be proficient in Oasis documentation.

Work Environment & Schedule

  • Dress code: Scrubs.
  • Weekend work: Approximately once per month; varies by location and must be discussed during interview.
  • On-call requirements: Yes, schedule to be determined; varies by location and profession.
  • Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard Monday-Friday 5x8.
  • No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged to pick up hours.
  • Local candidates accepted under the same terms as travelers.

Compliance & Documentation Requirements

  • Must complete a variety of background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required for certain states and must be current per state regulations.
  • Education verification required for nursing degree.
  • Drug screening: 10-panel drug test required.
  • Health documents required:
    • Physical exam within 12 months confirming good health and no communicable diseases.
    • Negative TB test or chest X-ray within 12 months (no pending results at start).
    • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
    • Seasonal flu vaccination or documented declination.
    • COVID-19 vaccination series or documented declination.
  • Must review and sign multiple compliance, confidentiality, privacy, and policy documents prior to start and annually if assignment extends beyond one year.
  • Must complete Home Health Core Care Policy Quiz and review Home Health Scenarios prior to start.
  • References: Two professional references required.
  • Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.

Additional Notes

  • Mileage reimbursed at IRS standard rate for travel between patient homes; commute to/from first and last patient is not reimbursed.
  • Contractor will report to the home clinic in Eugene, Oregon.
  • Productivity expectations: 28-30 weekly units.
  • Prior travel experience is not required for this position.

This position is ideal for experienced Registered Nurses with recent home health or hospice experience who are comfortable working independently in a travel/contract role with a moderate travel radius.
