Required Qualifications

• BCACP board certification
• PGY2 Ambulatory Care residency
• 4 years of direct patient care demonstrating medication management
• Texas State Board of Pharmacy license
• BLS
• Medication Test/Pharmacology Competency Exam, minimum 80%
• Competency Exam, minimum 80%
• Government-issued photo ID
• Background check
• Education verification of initial related degree and most recent degree
• State license verification
• Acknowledgement of Pass/Fail Testing
• Physical exam within 1 year of start
• TDAP required; TD not accepted
• Negative TSpot or QGold within 1 year of start; PPD not accepted
• TB questionnaire
• Rubeola, Mumps, and Rubella immunity by positive titer or 2 vaccines
• Hepatitis B immunity by titer, declination, or vaccine series
• Varicella immunity by titer, 2 vaccines, or history of having the disease
• Negative 10-panel drug screen within 30 days of start
• Two references verified within 1 year of start
• Skills checklist updated annually and not expired within 30 days of start

Additional Notes

• Most primary care clinics have extended daytime hours, 7a-7p
• If the candidate can only work 8-hour shifts but can still provide 40 total hours, help may be needed at another clinic
• Onsite role seeing patients as a mid-level provider
• Preceptor for pharmacy students or residents
• Participates in collaborative practice agreements, drug utilization reviews, medication action plans, patient education, and multidisciplinary team care
