LPN Ambulatory

Location

Rochester, Minnesota

Contract

13 weeks
36 hours per week
12-hour shifts
Day or night schedule: 6:00 AM–6:00 PM or 6:00 PM–6:00 AM
Every 3rd weekend required

Required Qualifications

Valid LPN / LVN license in Minnesota
LPN / LVN (1 year of experience)
LPN / LVN experience within the last year
Graduation from a school of Practical Nursing, or one year of LPN experience if graduation occurred more than two years ago
Experience in a Rehabilitation setting
Ambulatory Care experience
Long Term Care experience

Experience / Skills

Experience working in a team environment
Excellent verbal and written communication skills
Ability to work flexible hours, including days, evenings, nights, holidays, weekends, and on-call
Ability to adapt to unpredictable situations within the work setting
Computer skills

Preferred Experience

1 year of LPN experience within the last 5 years
Assisted Living and Short Term Rehab experience
Long term care experience

Role Summary

Provides nursing care to assigned patients under RN or other assigned supervisor
Collects data through observation and communicates information to support patient assessment and care planning
Delivers care according to the patient care plan, policies and procedures, and relationship-based care principles
May participate in committees or quality improvement projects
