Job Summary
Provide social services to patients and their families under general supervision. Responsibilities include psychosocial assessments, discharge planning, crisis intervention, counseling, and acting as a liaison between patients, families, hospital staff, and community agencies.
Essential Responsibilities
- Conduct discharge planning evaluations and psychosocial assessments.
- Counsel patients and families on diseases, treatments, social/financial issues, emotional stress, and other concerns.
- Provide crisis counseling related to child abuse, neglect, physical abuse, and other high social risk cases.
- Assist with discharge planning and referrals to community or post-acute care services.
- Support patients and families with adjustments to illness, treatment, or changes in functional status.
- Use specialized communication skills with children.
- Advocate for patients on informed decision-making, end-of-life care, goal setting, and ethical dilemmas.
- May supervise students or newly hired employees.
- Prepare statistics related to referrals and extended stays as needed.
Schedule
- 8-hour shifts, 40 hours per week.
- Workdays may be any day Sunday through Saturday.
- Assignments may be at multiple hospital locations.
Required Qualifications
- Master's Degree in Social Work from an accredited school.
- Valid Licensed Master Social Worker (LMSW) license in Oklahoma.
- Minimum of 1 year of experience as a Licensed Master Social Worker, including recent experience within the last year.
- Experience working as a Social Worker.

- 13 weeks
