Job Summary

Provide social services to patients and their families under general supervision. Responsibilities include psychosocial assessments, discharge planning, crisis intervention, counseling, and acting as a liaison between patients, families, hospital staff, and community agencies.

Essential Responsibilities

• Conduct discharge planning evaluations and psychosocial assessments.
• Counsel patients and families on diseases, treatments, social/financial issues, emotional stress, and other concerns.
• Provide crisis counseling related to child abuse, neglect, physical abuse, and other high social risk cases.
• Assist with discharge planning and referrals to community or post-acute care services.
• Support patients and families with adjustments to illness, treatment, or changes in functional status.
• Use specialized communication skills with children.
• Advocate for patients on informed decision-making, end-of-life care, goal setting, and ethical dilemmas.
• May supervise students or newly hired employees.
• Prepare statistics related to referrals and extended stays as needed.

Schedule

• 8-hour shifts, 40 hours per week.
• Workdays may be any day Sunday through Saturday.
• Assignments may be at multiple hospital locations.

Required Qualifications

• Master's Degree in Social Work from an accredited school.
• Valid Licensed Master Social Worker (LMSW) license in Oklahoma.
• Minimum of 1 year of experience as a Licensed Master Social Worker, including recent experience within the last year.
• Experience working as a Social Worker.

Contract Length

• 13 weeks
