Location: Dallas, California
Employment Type: Long Term Contract (13 weeks)
Schedule: Monday - Friday, 40 hours guaranteed per week
Start Date: ASAP

Coverage area includes Shasta Lake south to Orland, with possible assignments within a 60-mile radius of the facility.
Average caseload: 4-6 visits per day (may include PT/OT/SLP visits).
Overtime requires pre-approval.
2 weeks cancellation notice required.

Licensed Occupational Therapist in California.
Home Health and OASIS experience required, no exceptions.
Experience with Homecare Homebase documentation required.
If no prior Homecare Homebase experience, must complete an online HCHB course prior to start date (client consideration).
