Role
RN (Registered Nurse) in Case Management / Utilization Review, covering both discharge planning and UR.
Location
Peoria, Illinois
Contract Length
13 weeks
Schedule
- Days
- Monday through Friday, 8:00 AM to 4:30 PM
- 8 hours per day, 40 hours per week
Required Qualifications
- Valid RN license in Illinois
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS issued by American Heart Association
- Case Management (2 years of experience)
- Case Management experience within the last year
- Experience with discharge planning
- Experience with insurance authorization
- Experience with care coordination in acute hospital setting
- Long Term Acute Care experience
- Utilization Review experience
Preferred
- Healthcare setting experience
- Background in Long Term Acute Hospitals or Rehab
Work Setting
- Inpatient Physical Rehab or LTAC facility
Caseload
- 13 to 25 patients per day
Key Responsibilities
- Collaborate with patients, families, and the multidisciplinary team
- Develop and manage plans of care from admission through discharge
- Manage throughput and length of stay
- Coordinate interdisciplinary rounding, care coordination, family meetings, utilization review, insurance authorization, concurrent reviews, advanced directives, surrogacy, discharge planning, DME, home health vs outpatient referrals, post-discharge referrals, transportation, and resources
- Set up vent facilities when needed
- Determine discharge pharmacy needs, including anticoagulant resources, IV antibiotics, home infusion or outpatient arrangements, and dialysis referrals
Additional Details
- EPIC is used
- MCG is used for UR, but experience is not required
- Scrubs or business casual; no colors
- No holidays required
- Weekend coverage may involve being available for questions and can be flexible
- Orientation is 12 hours and is non-billable
