Licensed Practical Nurse (LPN/LVN) - Skilled Nursing Facility

Job Summary

Seeking an experienced Licensed Practical Nurse (LPN/LVN) for a skilled nursing facility connected to a hospital. The facility provides care primarily for elderly residents, including those with cognitive disabilities. The role involves working in various units including Memory Care and requires experience in large long-term care (LTC) settings.

Facility Details

• Skilled Nursing Facility with 79 beds (mix of private and semi-private rooms)
• Four care areas including a locked Memory Care unit
• Electronic Medical Record system: Point Click Care
• Nurse-to-patient ratios vary by unit; Memory Care staffed 24/7 with dedicated nurses and CNAs
• No telemetry monitors on site
• No on-site pharmacy; medication dispensing system not available
• Interdisciplinary team includes Social Worker, PT/OT, and Speech therapists (hospital therapy department)
• Physician available on-call after hours

Work Schedule

• Shifts: 8 or 12 hours starting at 6 AM, depending on need
• Weekend requirement: Every other weekend
• Holiday coverage as needed
• No call or standby requirements
• Floating within the facility’s halls and units (not hospital-wide)
• Orientation: 2 days in each unit for RNs (LPN orientation details not specified)

Required Qualifications

• Valid LPN/LVN license in Iowa
• Minimum of 1 year of experience in Long Term Care, including experience within the last year
• Successful completion of at least one prior travel nursing assignment; not a first-time traveler
• BLS certification issued by the American Heart Association
• Mandatory Reporter Training - Adult
• Iowa Dementia Training (8 hours annually)

Job Duties

• Provide nursing care consistent with LPN scope; similar duties as RNs except IV administration unless certified
• Assist with resident transportation to hospital departments as needed
• Participate in interdisciplinary care but no requirement to attend acute inpatient rehab team conferences
• Support Memory Care and other units with nursing and CNA staff

• Dress code: Any scrub color; no sweatshirts or t-shirts
• Facility is affiliated with a Catholic organization; daily mass available with volunteer assistance for residents
• Staff expected to assist residents with transportation and daily activities as needed

This position is ideal for LPNs with solid long-term care experience and prior travel nursing assignments who are comfortable working in a skilled nursing environment with elderly and cognitively impaired residents.
