Job Description

CVHCare is seeking a qualified Physical Therapy Assistant with a minimum of 1 year of professional experience. Home health experience is preferred.

Assigned Areas

• Area #10: San Mateo, Brisbane, Burlingame, Daly City, Redwood City, Hillsborough, Millbrae, Colma, South San Francisco, Pacifica, Palo Alto, Belmont, San Bruno, Half Moon Bay, San Carlos, Menlo Park, El Granada
• Area #9: San Francisco

Work Options

• Guaranteed Hours:

  • Minimum of 36 hours guaranteed.
  • Must complete at least 30 points per week (points system described below).
  • Willingness to travel outside the area or work weekends may be required.
• Non-Guaranteed Hours:

  • Weekend coverage may be required.
  • May need to go outside the service area occasionally to secure a full caseload.
  • No guaranteed hours; travelers can refuse patients.

Points System

• Routine Visit: 1 point = 1.33 hours
• Payment is made on a per-visit basis, covering drive/travel time, visit time, documentation time, and time spent on calls to patients and doctors.

Credentials Required

• Current California PTA License
• BLS Certification
• Driver's License
• Auto Insurance
• Auto Registration
• Annual TB Test
• MMR, Varicella, HepB, TDap (10 years), and Annual Flu Vaccination
• Drug Screen
• Physical Examination

Additional Information

• All requested time off (RTO) must be disclosed at the time of submission.
• Agencies must ensure that their contract mirrors the confirmation page to avoid removal from the client's vendor set.
